Medical and Prescription Drug Insurance (Blue Cross & Blue Shield of Alabama – Group #88544)

3Summary of Benefits and Coverage (SBC) – These high level benefit summaries are required to be made available under the Affordable Care Act and make it easy to compare benefit plans. Also included is the Uniform Glossary of Terms which are not specific to your Blue Cross and Blue Shield plan but is very helpful for understanding various terms and how your plan works.

Annual Benefit Notices – DOL, ACA, ERISA and other required notices for new hires and during open enrollment.

CYT – Alabama Plant’s Voluntary Wellness Program – In order to be eligible for the lower premium contribution starting on 4/1/24, Associates must complete 2 of the 3 options between 1/1/23 and 12/22/23:

Option 1 (Only applies to the Associate, not your covered spouse or children)

  • Provide proof of being, “fully vaccinated as defined by the CDC” as of the day you submit a copy of your vaccine card to Human Resources.

Option 2 (Applies to Associates and your covered spouse)

  • A biometric screening
  • We anticipate the annual screening event being held onsite again in October.

Option 3 (Applies to Associates and your covered spouse)

CYT – Alabama Plant handles the administration and notification of your rights under COBRA and ability to continue the insurance if a qualifying event. Please notify Tonya Perkins in Human Resources of any qualifying events so she can in turn notify the COBRA administrator.

Non-Network medical claim form – In-network providers are responsible for submitting any claim forms to Blue Cross & Blue Shield of Alabama on your behalf. However, if you have services provided by a non-network provider, you will need to pay that provider directly and complete this form to be reimbursed.

Authorization Form – Blue Cross & Blue Shield of Alabama is not authorized to release protected health information without your consent. In most instances verbal authorization can be given to our insurance agent, Milestone Benefits Agency, Inc., for them to assist you or your dependent. However, in certain instances written authorization must be given to Blue Cross & Blue Shield of Alabama. If you want to authorize someone to have access to your information you must complete this form and send it to Blue Cross & Blue Shield of Alabama.

Online Provider Lookup – From the home page select, Find a Doctor“.  On the next page, select the Preferred Provider Organization” network for doctors, facilities, labs, etc.  For behavioral/mental health providers select the, “Expanded Psychiatric Services” network.  If you’re traveling in any of the other 49 states outside Alabama, select the national network and enter “PPA” as the contract prefix.

myBlueCross– Blue Cross & Blue Shield of Alabama’s online member access site.   Once you’ve registered you can access claim statements, order replacement ID cards, view claim history, manage your prescription drugs, find providers and much more.  Review the two page flyer for more information.

Health and Wellness – Blue Cross & Blue Shield of Alabama provides online health resources and wellness programs for diabetes, cardiac and respiratory conditions. Other popular programs include:

Blue365 – Online healthy discounts and deals with top brands such as Fitbit, Jenny Craig, Reebok and more.

BeHealthy.com – A comprehensive health website powered by WebMD®, BeHealthy.com provides you with up-to-date information about illnesses, conditions and the latest health news.  You also get access to health trackers, calculators, health assessments, and can personalize the site to fit your needs.

Baby YourselfBaby Yourself Maternity Program helps ensure expectant mothers and their babies receive the best possible healthcare during pregnancy. This program is available to expectant mothers, regardless of whether or not their pregnancy is normal or high risk.

Dental Insurance (Delta Dental of Ohio – Group #0541)

Dental Claim Form – In-network dentists will complete and send your claims to Delta Dental on your behalf. However, if you use a non-network dentist you may have to pay up front, complete this form and send it to Delta Dental at the address below:

Online Provider Lookup – Be sure to select either the “Delta Dental PPO” or “Delta Dental Premier” network.

Reference ID cards – These can be printed and given to your dentist when you visit them.

Aevsis Member Website – Here you can find helpful information on how to use your benefits, check your eligibility, view your benefits, locate participating providers, print ID cards, get answers to frequently asked questions and more.

Provider Lookup – Locate providers online within a radius of your zip code.  However, some providers may not participate in all plans. Therefore, Avesis recommends that you log into their website (see above) to see the current listing of participating providers included in your plan.

Group and Supplemental Life/AD&D Insurance (Mutual of Omaha – Group#G000B6S6)

Group Term Life and AD&D (Paid by CYT – Alabama Plant)

$50,000 per Associate (up to age 64 with reductions at age 65, 70 and 75)

Associate Supplemental Term Life and AD&D:

  • Associates: $20,000 to $500,000 in $10,000 increments not to exceed 5 times basic annual earnings.
    • Guarantee issue (no medical questions) up to $100,000.
    • For amounts above $100,000 you must complete an Evidence of Insurability questionnaire.
    • Age reductions after age 64 apply.
  • Spouse: $10,000 to $250,000 in $5,000 increments to 100% of the total amount of the Associate’s coverage.
    • Guarantee issue (no medical questions) up to $25,000.
    • The spouse’s rate is based on the Associate’s age.
    • For amounts above $25,000 you must complete an Evidence of Insurability questionnaire.
    • Coverage terminates when the Associate turns age 70.
  • Child(ren): $2,000 to $10,000 in $2,000 increments to age 26.
    • Guarantee issue (no medical questions) up to $10,000.

Employee Assistance Program (EAP) – Free assistance to you and your eligible dependents with various personal or job-related concerns.  Unlimited telephonic access 24 hours a day, seven days a week in addition to three face-to-face sessions with a counselor (per household per calendar year). Worldwide Travel Assistance – If you plan on traveling more than 100 miles away from home for work or pleasure, this free service provides 24/7 access to interpreter services, document replacement, locating legal services,  lost or stolen baggage and other services.  Just print the flier and take it with you when you travel.

 Hearing aid discount program – Call amplifon at 888.534.1747 to speak with a Patient Care Advocate who can answer any questions and assist you with finding a provider near you.

 Free Will Prep Service:

Mutual of Omaha provides a totally FREE Will Prep service for you, your legal spouse and adult children to the age of 26 for anyone covered under their group life or voluntary life insurance plans.

Documents include: Last Will and Testament for single/married persons, Power of Attorney, Healthcare Directive and Living Trusts.

Evidence of Insurability – For late enrollees or those electing coverage amounts in excess of the Guarantee Issue levels.

Conversion and Portability – When you leave CYT – Alabama Plant you can convert (Conversion Application) the group or supplemental term life insurance to an individual whole life plan.  You can also “Port” (Portability Application) your supplemental term life insurance.  This allows you to maintain the coverage as a term life benefit which is typically less expensive but increases in cost over time.

Short Term (STD) & Long Term Disability (LTD) (Mutual of Omaha)

Short Term Disability Benefits:

Hourly: (Summary Overview, Certificate Summary and Certificate) In the event you become disabled and have a minimum loss of 1% of your earnings, your short-term disability benefits start on the 8th day of an accident or illness.  The plan will pay you 60% of your base weekly earnings, not to exceed $350 per week for up to 12 weeks. This benefit is paid for by CYT – Alabama Plant at no expense to Associates.

Technical Specialists:  (Summary Overview, Certificate Summary and Certificate) In the event you become disabled and have a minimum loss of 1% of your earnings, your short-term disability benefits start on the 8th day of an accident or illness.  The plan will pay you 60% of your base weekly earnings, not to exceed $500 for up to 12 weeks. This benefit is paid for by CYT – Alabama Plant at no expense to Associates.

Exempt: (See the CYT – Alabama Plant Handbook) In the event you become disabled, the plan will pay 100% of your base weekly earnings for the first 9 weeks and then 60% of your base weekly earning for the remaining 4 weeks.

Long Term Disability Benefits (Summary Overview, Certificate Summary and Certificate):

For all Full-Time Associates, after satisfying a 90 day elimination period (the end of the Short Term Disability Benefit), the plan will pay 60% of your basic weekly earnings to a maximum of $5,000 for up to 2-years.  Benefits may be reduced by other sources of income and some disabilities may not be covered or may have limited coverage under this policy.  If you become disabled prior to age 60, the maximum duration of payments will be five years.  If you become disabled after turning 60 then there is a schedule in your benefit booklet that shows the maximum duration you could be paid.

How to file a claim: Since CYT – Alabama Plant provides both a Short and Long Term Disability benefit, all claims begin under the Short Term Disability benefit. To initiate a disability claim, follow the steps outlined in this link:

Step1: Notify your supervisor
Step2: Call MoO at 800.877.5176 Opt.1
Step3: Take forms to your doctors

If it appears your disability will extend beyond 180 days, your STD case worker will contact you and your physician to assist you with the LTD application process. In all cases, you must contact the Human Resources department and your supervisor to notify them of your status. You must also complete the Waiver of Premium Claim Form so your LTD benefits and premiums can continue without any further premium payments.
